Request Medical Records
Visit any of the three convenient Tucson Orthopaedic Institute locations to make a request for medical records. The Medical Records Correspondence main window is located in the clinic of the East office, at 5301 E. Grant Road in the TMC Orthopaedic and Surgical Tower.

How to Obtain Your Records
Tucson Orthopaedic Institute takes patient privacy seriously; therefore, we require that you present a photo ID when making a request or picking up your medical record.
To authorize your medical records release, you may fill out the Authorization for Medical Records Release of Health Information Form (PDF) and return it to the Tucson Orthopaedic Institute office nearest you.
For faster access without a fee, you can now access your Tucson Ortho health record via our patient portal 24/7. Set up your secure myhealth@TucsonOrtho account to view, download, fax, or email select health information. (Note: Images such as X-rays or MRI cannot be downloaded from the portal.)

If the records you need are not available electronically, it will take us 5 to 7 days to process your request. Factors such as size of the record and the volume of the requests will affect the response time. If you have not heard from us after 7 days, please call (520) 784-6179.
Note: Urgent need circumstances demanding immediate attention will require that the patient arrive at the Medical Records office to complete the necessary paperwork for the release of the records.
Medical Record Fees
Tucson Orthopaedic Institute will charge a fee for the cost of reproduction of medical records and images. Please contact Medical Records Correspondence at (520) 784-6179 for current record/images fee information.

Tucson Orthopaedic Institute is committed to protecting the security and privacy of its patients. All information contained within the patient medical record is considered confidential and is federally protected under the Health Insurance Portability and Accountability Act of 1996 (HIPAA).
Medical records will not be released without the written authorization of the patient, his or her representative, a subpoena, or court order.
